About the role
- Coordinate patient care and ensure patients receive appropriate services in a timely manner
- Advocate for patients' health needs
- Manage acute care situations and support patients through their healthcare journey
- Patient assessment and case management in hospital medicine
- Perform care coordination, discharge planning, and needs assessment
- Handle prior authorizations, appeals and denials, and benefits eligibility
- Participate in utilization review and admission criteria evaluation
- Manage worker's compensation case management and regulatory compliance (CMS, DRG, HIPAA, The Joint Commission)
