About the role

  • Provide day-to-day management and oversight of Palliative Care Delivery teams
  • Lead interdisciplinary teams of Providers, RNs, Social Workers, and Medical Assistants
  • Develop and implement program strategies, goals, and initiatives
  • Drive execution and achievement of department KPIs
  • Assess palliative care referrals based on patient and family needs
  • Ensure delivery of coordinated, patient-centered care
  • Partner with department leaders across the organization

Requirements

  • Master of Science in Nursing (MSN)
  • Active, unrestricted Nurse Practitioner (NP) license required; ability to obtain California NP licensure within 90 days of hire (if unlicensed in CA), plus additional state licensure as needed.
  • Board-Certified Nurse Practitioner (ACHPN certification preferred)
  • 5-8+ years of progressive healthcare management experience in fast-paced practice environments
  • Proficiency in Salesforce and Google Suite applications
