About the role
- Provide day-to-day management and oversight of Palliative Care Delivery teams
- Lead interdisciplinary teams of Providers, RNs, Social Workers, and Medical Assistants
- Develop and implement program strategies, goals, and initiatives
- Drive execution and achievement of department KPIs
- Assess palliative care referrals based on patient and family needs
- Ensure delivery of coordinated, patient-centered care
- Partner with department leaders across the organization
Requirements
- Master of Science in Nursing (MSN)
- Active, unrestricted Nurse Practitioner (NP) license required; ability to obtain California NP licensure within 90 days of hire (if unlicensed in CA), plus additional state licensure as needed.
- Board-Certified Nurse Practitioner (ACHPN certification preferred)
- 5-8+ years of progressive healthcare management experience in fast-paced practice environments
- Proficiency in Salesforce and Google Suite applications
