About the role
- Oversee clinical operations of the home care agency and ensure regulatory compliance
- Supervise and support Field RNs including documentation review, training, orientation, and competency evaluations
- Coordinate patient care and conduct clinical assessments for NHTD and TBI Waiver programs
- Participate in team meetings and resolve incidents or complaints
- Conduct audits, surveys, and performance improvement initiatives to ensure NYSDOH and CMS compliance
- Identify gaps in care, implement corrective actions, and monitor incident reports to promote quality assurance and risk management
- Serve as clinical resource and liaison for staff, patients, families, caregivers, and physicians
- Support the Director of Patient Services with projects and maintain high standards of patient care and safety
Requirements
- Current New York State Registered Nurse (RN) license in good standing
- Minimum of 3 years of nursing experience
- 1–2 years in home care and/or NHTD Waiver preferred
- Excellent organizational, leadership, and communication skills
- Proficiency in EMR systems and Microsoft Office Suite
