About the role
- Focus on the care management of patients with acute and chronic medical conditions
- Assess care needs of vulnerable populations for various clients
- Collaborate with health care providers to assure coordination of care
- Provide ongoing education and support to patients to improve health management skills
- Document per contractual requirements related to individual member status
Requirements
- Four-year degree in health care or related field and/or equivalent training and/or experience
- Current unrestricted RN license required
- Minimum 1-year experience in human services and/or 3 years of clinical practice experience
- Ability to work 8:30 am to 5:00 pm
- Case management and/or Mental Health experience preferred
- Certified Case Manager (CCM) strongly preferred or ability to obtain CCM once becoming a Telligen employee-owner
