About the role
- Serve as the primary point of contact for patients enrolled in the Collaborative Care program.
- Support the mental and physical health of patients on an assigned caseload, coordinating closely with the patient’s medical provider and, when appropriate, other mental health professionals.
- Participate in regular caseload consultations with the Psychiatric Consultant and communicate resulting treatment recommendations to the patient’s medical provider.
- Screen and assess patients for common mental health and substance use disorders using standardized tools, and facilitate patient engagement and follow-up care.
- Provide education to patients on common mental health and substance use disorders, including available treatment options.
- Deliver brief, evidence-based behavioral interventions such as behavioral activation, problem-solving treatment, motivational interviewing, and other appropriate therapies.
- Track treatment response, monitor changes in clinical symptoms, and identify any treatment side effects or complications.
- Support psychotropic medication management prescribed by medical providers, focusing on adherence, effectiveness, and side-effect monitoring.
- Track patient follow-up and monitor clinical outcomes.
- Document patient progress and treatment recommendations in the EHR and other required systems, ensuring information is shared with medical providers, the Psychiatric Consultant, and other treating professionals.
- Adjust treatment plans for patients who are not improving as expected in consultation with the medical provider and Psychiatric Consultant, including referral to more intensive or specialized care when needed.
- Facilitate referrals to evidence-based psychosocial treatments and other clinically indicated services outside the organization.
- Develop relapse prevention and self-management plans for patients who have achieved their treatment goals and are preparing to be discharged from the caseload.
Requirements
- Bachelor's degree in social work or other behavioral health related degree
- Specialized training to provide brief psychosocial interventions, motivational interviewing, and problem-solving treatment required.
- Minimum of 2 years of experience in behavioral health, integrated care, or care management.
- Clinical License preferred: Licensed Mental Health Counselor/Professional Counselor, Licensed Marriage and Family Therapist, Licensed Social Worker, Licensed Psychologist, Master’s-level licensure candidate/trainee (e.g., LMSW)
- Demonstrated ability to collaborate and communicate effectively in a team setting.
- Experience with screening for common mental health and/or substance abuse disorders.
- Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
- Experience with evidence-based counseling techniques
