About the role
- Responsible for leading and overseeing the home health and hospice hospital liaison team.
- Support seamless transitions from inpatient care to post-acute care services.
- Ensure that patient referrals, admissions, and discharges are coordinated efficiently and compassionately.
- Maintain compliance with regulatory standards and organizational policies.
- Serve as a strategic partner to hospital leadership, physicians, and interdisciplinary teams.
Requirements
- 5 years recent relevant experience
- Extensive experience in post-acute care.
- Strong clinical knowledge of post-acute care.
- Familiarity with reimbursement methods and regulatory requirements.
- Proficiency in EMR systems and documentation standards.
- Ability to work in a fast-paced, emotionally demanding environment.
