About the role
- Accountability for the achievement of optimal outcomes within the standards of practice.
- Demonstrates professionalism by emulating the standards of practice of care management guided by the Shriners Children's professional practice model FOCUSED.
- Collaborates by working with patients, families, caregivers, and the health care team locally and within the Shriners Children's system to jointly communicate, problem solve, and share accountability for optimal outcomes.
- Coordinate the plan of care to achieve desired outcomes and integrates the work of the health care team, including the patient's Primary Care/Medical Home by coordinating resources and services necessary to accomplish agreed upon goals.
- Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.
- Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
Requirements
- Required BSN
- 3 Years Nursing experience
- RN License in state of practice
- Case Management Certification (CMC, ACM or ANCC), required within 12 months of hire or within 12 months of eligibility
- Preferred Masters in Nursing, or other Health-care Related Field
- 1 Year Care Management in pediatrics
