Demonstrates professionalism by emulating the standards of practice of care management guided by the Shriners Children's professional practice model FOCUSED.
Collaborates by working with patients, families, caregivers, and the health care team locally and within the Shriners Children's system to jointly communicate, problem solve, and share accountability for optimal outcomes.
Coordinate the plan of care to achieve desired outcomes and integrates the work of the health care team, including the patient's Primary Care/Medical Home by coordinating resources and services necessary to accomplish agreed upon goals.
Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.
Advocates by supporting or recommending services for the protection of the patient's health, safety and rights.
Requirements
Required BSN
3 Years Nursing experience
RN License in state of practice
Case Management Certification (CMC, ACM or ANCC), required within 12 months of hire or within 12 months of eligibility
Preferred Masters in Nursing, or other Health-care Related Field
1 Year Care Management in pediatrics
