💰 $73 - $103 per hour
About the role
- provides assessments, planning, interventions and follow-up for patients and family members
- addresses potential or actual clinical and/or psychosocial needs
- facilitates adherence to a treatment plan to optimize patient health
- collaborates with the interdisciplinary team to ensure safe and seamless transitions of care
- documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements
- maintains current knowledge of pediatric specialty practice, healthcare economics, trends, and reimbursement methodologies
Requirements
- Bachelor in Nursing (BSN)
- 3 years nursing experience
- Current California RN license
- Case Management Certification (CMC, ACM or ANCC) required within 12 months of hire or eligibility
- Master's in Nursing, or other Healthcare Related field preferred
- 1 year Care Management in Pediatrics preferred
- 5 years clinical experience in one or more of the facility's service lines (SCL, Pediatric Orthopedic Burn Care, cleft lip and palate) preferred
