Perform OT evaluations and develop and implement OT plan of care in accordance with PACE policies.
Serve as a member of the Interdisciplinary Team and communicate and coordinate care with the team, outside providers, caregivers and family members.
Function independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assisted Living Facilities and nursing homes.
Assume responsibility for participants treated by the OTA and OT Aide.
Work 32 hours/week on weekdays, day shift, providing clinic and home visits to support elders in the community.
Requirements
Bachelor's Degree in Occupational Therapy.
Within 30 days of hire: National Provider BLS - American Heart Association.
Upon hire: Washington Occupational Therapist License.
Preferred: Master's Degree in Occupational Therapy or Ph.D. in Occupational Therapy.
Preferred: 1 year of experience working as an occupational therapist in a geriatric setting.
Ability to work 32 hours/week on weekdays, day shift.
