Assesses needs and provides Certified Occupational Therapy Assistant (COTA) services and direct treatment in consultation with the Occupational Therapist.
Functions as a member of the Interdisciplinary Team.
Responsible for home care coordination.
Provide clinic and home-visit therapy to help older adults live independently in the community.
Coordinate care within the Providence PACE program, including clinics and home visits.
May travel to patient homes (mileage reimbursement provided).
Requirements
Graduate of a Certified Occupational Therapy Assistant program.
National Provider BLS - American Heart Association within 30 days of hire.
Oregon Occupational Therapist Assistant License upon hire.
Preferred: 1 year - Experience in the area of Occupational Therapy as a COTA.
