About the role
- Assesses needs and provides Certified Occupational Therapy Assistant (COTA) services and direct treatment in consultation with the Occupational Therapist.
- Functions as a member of the Interdisciplinary Team.
- Responsible for home care coordination.
- Provide clinic and home-visit therapy to help older adults live independently in the community.
- Coordinate care within the Providence PACE program, including clinics and home visits.
- May travel to patient homes (mileage reimbursement provided).
Requirements
- Graduate of a Certified Occupational Therapy Assistant program.
- National Provider BLS - American Heart Association within 30 days of hire.
- Oregon Occupational Therapist Assistant License upon hire.
- Preferred: 1 year - Experience in the area of Occupational Therapy as a COTA.
