RN Fellow will experience a guided transition into the Care Manager specialty through structured curriculum, experienced preceptors and hands-on learning via the Providence Clinical Academy.
Provide care management including care coordination, transitional care management, and facilitating communication among patients, caregivers, nurses, social workers, physicians, payers, support staff, other practitioners and the community.
Recognize and respect the patient's right to self-determination and address social determinants of health and complexities of care.
Collaborate with multidisciplinary teams to support effective transitional care management and continuity of care.
Participate in full-time day shift fellowship starting November 3, 2025 at Providence Cedars-Sinai Tarzana Medical Center.
Requirements
Bachelor’s Degree in Nursing, preferred (All Registered Nurses without a minimum of a Bachelor’s degree in Nursing must obtain a Bachelor’s degree in Nursing or higher within 3 years of hire).
Upon hire: California Registered Nurse License.
California Fire and Life Safety Card - National Organization within 30 days of hire.
2 years experience in Healthcare related field (Acute, Ambulatory, Post-Acute, etc.).
2 years experience in Case Management (Care Coordination or Utilization Management) or successful completion of the Transitions in Practice (TIP) program for Care Manager (TIP candidates must have experience in same type of nursing unit in which the CM position is available).
More than 12 months of continuous RN work experience to participate in the Providence Clinical Academy.
Preferred: Master's Degree in a healthcare related field.
Preferred: Upon hire: Certification in Case Management (ACM or CCM).
