About the role
- Leads the alignment, standardization and ongoing improvement of ministry length of stay for designated patient populations
- Serves as designated ministry liaison with providers and ministry Care Coordination teams
- Plans and develops reporting tools for sharing of information between division and ministry teams
Requirements
- Associate's Degree - Nursing
- Bachelor's Degree - Nursing
- California Registered Nurse License upon hire
- 5 years - Experience as a utilization/case manager in an acute care setting (preferred)
- Experience in a multi-hospital and/or integrated healthcare system (preferred)
