About the role
- Care Management is a collaborative practice model including the patients, nurses, Care Manager LCSWs, physicians, other practitioners, caregivers and the community
- The care management process encompasses excellent communication, both verbal and written, and facilitates care along a continuum through effective resource coordination and addressing the psych social needs of the patient
Requirements
- Master's Degree in Social Work
- Upon hire: California Clinical Social Worker License
- 2 years of relevant Social Work experience in a medical group, health plan, hospital, community, post-acute, or medical office/clinic setting
- Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving
