About the role
- Apply advanced skills to provide care management services to patients
- Connect patients with healthcare providers and resources
- Provide patient-centered and trauma-informed care
- Collaborate with a team of physicians, clinicians, and RNs
- Review lab results and diagnostic imaging/studies
- Develop and evaluate care plans for high-risk patient cases
- Discuss treatment plans and provide health education
- Document accurately and thoroughly
- Evaluate the effectiveness of care plans
- Integrate equity and inclusion best practices into all job functions
Requirements
- Registered Nurse licensure in Michigan
- Two or more years of clinical case management, care management, and/or care coordination experience, preferably in an ambulatory/outpatient setting
- Caring, professional demeanor and excellent customer service skills
- Excellent critical thinking, assessment, and problem-solving skills
- Ability to work independently with minimal supervision
- Experience working in Electronic Medical Records, especially Epic
- Ensure protection of patients’ privacy and Protected Health Information (PHI) in a home office environment
