About the role
- Provide direct behavioral health and case management services to adults and families with complex needs, including ACT-informed practices
- Travel to client homes and within communities to deliver services
- Provide outpatient competency restoration education for individuals deemed incompetent by the court
- Conduct brief initial behavioral health screenings to determine eligibility and treatment needs
- Offer crisis support including immediate non-clinical de-escalation, ensure safety, and coordinate with clinical team or crisis services
- Lead or co-facilitate psychoeducational groups focused on skill-building
- Co-facilitate or lead non-clinical substance use interventions and individual support under supervision
- Collaborate in a shared caseload model to ensure consistent care across the team
- Support clients in developing and achieving treatment goals using motivational interviewing and strengths-based practices
- Assist with activities of daily living, skill building, and developing home routines
- Provide individual and/or group Outpatient Restoration Services (Education and Case management)
- Coordinate and navigate social service systems such as Medicaid/Medicare, housing supports, and Medicaid waivers
- Maintain thorough documentation of all service-related contacts and develop service and transition plans
- Provide community-based services in natural settings and apply ACT principles such as assertive engagement and integrated care
- Assist clients with housing navigation, applications, barrier mitigation, and accompaniment to housing appointments when necessary
Requirements
- Bachelor's degree in Social Work, Psychology, or a related behavioral health field (required)
- Minimum one year of relevant experience in behavioral health and/or case management (required)
- Experience working with individuals with co-occurring mental health and substance use conditions and/or ACT teams (strongly preferred)
- Experience with housing navigation (a plus)
- Spanish-speaking / bilingual candidates preferred
- Valid driver’s license and auto insurance
- Must live in or near the community served
- Strong written and verbal communication skills
- Understanding of HIPAA, confidentiality, and protected health information
- High level of independence, time management, and ability to prioritize multiple tasks
- Commitment to cultural awareness, equity, and working with diverse populations
- Strong conflict resolution and decision-making skills
- Familiarity with motivational interviewing
