- Work in partnership with patients, families, caregivers and providers to deliver targeted care
- Collaborate with interdisciplinary staff and stakeholders within the care continuum
- Promote patients’ optimal health through disease prevention and management strategies
- Apply evidence-based interventions for cost efficient and outcome-based care
Requirements
- Bachelor's Degree (Required)
- APRN - Advanced Practice Registered Nurse License - State of Ohio Board of Nursing
- BLS - Basic Life Support - American Heart Association
- RN - Registered Nurse - Ohio Board of Nursing
- 2-5 years recent nursing practice experience
- Knowledge of acute hospital, community or public health nursing concepts and resources
- Computer skill and data entry
- Knowledge and skill in clinical assessment and disease management
