About the role
- Ensures organizational excellence related to customer service and quality of care
- Completing biopsychosocial assessments, psychiatric diagnoses, formulation of treatment plans and therapy sessions
- Develops care plan, including discharge plan
- Provides crisis intervention, information and referral and linkage to community resources
- Coordinates clinical and financial outcomes of an assigned population
- Thoroughly documents assessments and interventions in patients medical record
Requirements
- Master's Degree (Required)
- DL - Driver's License - Department of Motor Vehicles
- LSW - Licensed Social Worker - Social Work Certification and Licensure Board
- Interview, assessment, organizational and problem solving skills
- Ability to identify appropriate community resources on assigned caseload and to work collaboratively with patients, families, multi-disciplinary team and community agencies to achieve desired patient outcomes.
- Demonstrates patience and tact when dealing with patients, families and other staff.
