Care Coordinator

• Perform work consistent with NYAP’s mission, values, and philosophies
• Complete required training
• Develop and implement recruitment strategies for viable licensed foster homes
• Support potential foster parents in understanding youth needs, mentoring primary families, and reunification goals
• Recruit new foster families with current, former, and prospective foster parents
• Implement foster care rules, regulations, and policies
• Conduct face-to-face meetings with potential families within the first two weeks
• Assist families with required information and licensing paperwork
• Help foster parents overcome licensing barriers
• Develop and submit comprehensive home studies for approval
• Maintain and update foster home records
• Provide ongoing training and support to licensed foster homes
• Complete annual evaluations and recertification
• Support foster parents through information, education, meetings, support groups, and one-on-one discussions
• Travel as needed to provide community-based services to youth and families
• Participate in Continuous Quality Improvement activities
• Participate in weekly supervision
• Report incidents to state and supervisory agencies when appropriate

Foster Care Licensing Coordinator

Therapist

• Perform all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies
• Develops, implements, and supervises behavioral plans according to the needs of the program, staff, families, and persons served
• Ensures all relevant school personnel, family service counselor, foster parents or guardians and bio parents (as needed) are educated and instructed on individual behavioral plans for persons served
• Establishes and maintains contact with relevant community and school personnel to provide accurate monitoring
• Maintains accurate and up to date records of evaluations, behavioral plans, treatment plans, and progress
• Effectively and appropriately sets limits with persons served that are appropriate to the situation
• Transports persons served as needed
• Models appropriate and professional behavior
• Ensures that documentation of progress notes, treatment and behavior plans, and assessments are accurate and completed in the required time
• Relates with persons served, foster parents, guardians, community, partners, peers and the general public in a respectful and professional manner in person and phone interaction
• Performs other duties as assigned.

Facilitator

• Family facilitation, mediation, or conflict resolution.
• Strengths-based assessment and planning.
• Communication and relate to diverse groups and manage interpersonal conflict.
• Perform all duties in accordance with personnel policies and fiscal guidelines.
• Perform other duties as requested.

CSED Wraparound Facilitator

Implementation Specialist

• Coordinate IT activities for new office openings, office relocations, closures, expansions, and other facility-related projects
• Serve as the primary IT coordination point for Facilities, State Directors, local people in charge, internal IT teams, vendors, contractors, and other stakeholders
• Create and maintain project plans, schedules, implementation checklists, milestones, dependencies, action items, and completion dates
• Gather and document technology requirements for each location, including internet connectivity, network equipment, wireless access, phones, printers, copiers, security systems, cabling, and other IT-related equipment and services
• Coordinate internet service provider activities, including service availability, quotes, orders, installation appointments, circuit activation, and issue resolution
• Coordinate with network, security, and technical support teams to ensure equipment and services are configured, installed, tested, and ready for use
• Coordinate copier, printer, low-voltage cabling, security, and other technology vendors from initial request through project completion
• Obtain and compare vendor quotes and support the selection of cost-effective solutions that meet technical and operational requirements
• Track equipment orders, service requests, shipping, delivery, installation, configuration, testing, and deployment
• Confirm that all technology requirements are completed before an office opening, move, closure, expansion, or project go-live
• Proactively identify project risks, issues, delays, dependencies, and resource needs and escalate concerns to the appropriate leadership
• Monitor project milestones and update schedules, priorities, and stakeholder expectations when delays or changes occur
• Provide clear and timely project status updates, including upcoming activities, assigned responsibilities, risks, decisions, and completion dates
• Schedule and facilitate project meetings, vendor calls, readiness reviews, and implementation checkpoints
• Document meeting notes, decisions, action items, owners, and due dates and follow up until items are completed
• Maintain accurate project records, office technology inventories, vendor information, service details, implementation documentation, and reports
• Ensure office closures include the proper cancellation, transfer, return, removal, or disposal of IT equipment and services
• Conduct post-implementation reviews to verify that services are functioning properly and outstanding issues are documented and resolved
• Recommend improvements to project processes, templates, checklists, documentation, and implementation standards
• Maintain positive and professional working relationships with staff, vendors, contractors, and internal departments
• Perform other related duties as assigned
