About the role
- Meet with the patient/family/decisionmakers
- Educate on the HomeHealth service line and manage referral source/patient expectations
- Capture discharge paperwork as needed for a safe and quality transition
- Communicate with the local team to ensure all orders are in place and Start of Care appointments scheduled
- Communicate with the referring partner on any barriers or needs before discharge
- Ensure there is a physician willing/able to follow for care
- Schedule or ensure a post discharge follow up appointment is scheduled with the patient’s PCP
- Complete a24-48 hour follow up call to ensure the patient received care and risk stratify potentially rehospitalization
- Document in Point Care under CCIN (Clinical Comment Intake Note and CTC Follow Up note) regarding patient engagement/interaction when applicable
- Build and maintain strong relationships with all stakeholders
- Conducts business in a manner that displays high-level confidentiality, and integrity at all times
- Additional responsibilities as assigned
Requirements
- Licensed Vocational Nurse with active license in the state of employment
- Must have a valid driver’s license and auto insurance meeting state minimums
- Robust knowledge of referring partner and market dynamics, as well as knowledgeable regarding transitions of care
- Knowledge of home health care marketing or hospice management
- Understands the Federal, State, and local laws and regulatory guidelines governing home health and hospice operations
- Excellent at communication and communicating within a team environment with the ability to interact with a diverse group of individuals
- Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
- Must be able to function efficiently in a stressful work environment
- Able to manage conflicting priorities and proactively communicate with direct leadership
- Previous experience in home health or hospice is preferred
