About the role
- Assist patients with various financial and economic needs
- Update team with important issues to patient needs that assist with overall health
- Stay up to date with knowledge in this space
Requirements
- Bachelor's Degree (preferred)
- Associates Degree (required)
- 1+ Years in outpatient Behavioral Health field and/or Social Support field
- 2+ Years in direct care management position conducting biopsychosocial assessments and care plan development (preferred)
- Understanding of how to navigate the Federal and State programs.
- Must possess a good understanding of disability, food stamps, and IHSS
- Comfortable managing more than 80 patients
- Bilingual (preferred)
- 3 weeks (2 weeks + 6-7 federal holidays) paid time away
