About the role
- Reviews patient files and Registry information to make appropriate Care Plan recommendation.
- Effectively communicates patient issues and care plan recommendations to Provider and Psychiatrist plus implement their recommendations.
- Promotes patient engagement and robust education.
- Provides leadership for CoCM team members to improve patient outcomes.
- Performs psychosocial assessments using evidence-based, standardized behavioral health tools and protocols.
- Participates in Care Team Conferences.
Requirements
- Bachelor’s Degree (Required)
- 2+ years of experience in behavioral healthcare management (required)
- Strong knowledge of federal and state programs
- Bilingual candidates encouraged to apply
- Paid Time Away: 3 weeks (2 weeks + 6-7 federal holidays)
