Manager

• Manage patients’ specialized needs based on their individual condition(s) and/or reason for recent utilization in collaboration with physicians, advanced practice providers, and care coordination team members.
• Take full ownership of assigned patients with complex chronic conditions, serious illness, advanced frailty, or recent healthcare utilization with the goal of avoiding preventable admissions, reducing unnecessary healthcare use, and optimizing patient outcomes.
• Work with the patient to identify needs based on their values, goals and preferences through standardized assessments and workflows.
• Develop an effective, and comprehensive plan of care for each member in partnership with the patient.
• Coordinate patient care delivery with Honest clinicians, network providers, contracted vendors, and community-based services.
• Quickly build empathetic relationships with patients and families.
• Evaluate and identify patients’ needs based on their respective values, goals, and preferences, then translate these needs into clinical needs.
• Interface with primary care physicians, advanced practice providers (APP), specialists, and various disciplines on the development of case management plans/programs.
• Implement care/treatment plans by coordinating access to health services across multiple providers/ disciplines in conjunction with the physician/APP.
• Refer patients to internal Honest team members for care management activities as outlined by defined procedures.
• Monitor care and identify cost-effective measures, including recommendations for alternative levels of care and utilization of resources.
• Coordinate patient admissions and discharges from hospitals or skilled nursing facilities via coordination with respective facility clinicians and case managers.
• Build relationships across network hospitals, SNFs, home health companies, and DME vendors within the respective community.
• Monitor and evaluate the effectiveness of the case management plans based on quality and cost drivers and modify as necessary.
• Coordinate the interdisciplinary approach to providing continuity of care, including transfer coordination, discharge planning and obtaining all authorizations/approvals/transfers as needed for outside services for patients/families.
• Act as a patient advocate and educator to assure that the patient has the knowledge to care for his/her condition and patient is educated and empowered to be responsible for participating in the plan of care.
• Develop individualized patient/family education plan focused on self-management and deliver patient/family education specific to a disease state.
• Engage internal resources to identify and respond to social determinants of health such as lack of transportation, stable housing, or food resources.
• Participate in data collection and analysis of clinical outcomes of care and customer satisfaction standards. Participate in the formulation and implementation and monitoring of action strategies and outcomes of care or customer service. Ensure that accurate records are maintained of the care associated with each patient.
• Actively participate in huddles, interdisciplinary team (IDT) sessions, and patient case conferences.

• Oversee the technical strategy and delivery of complex, cross-functional technical programs for internal and external customers.
• Collaborate with business owners and technical leaders to define deployment scope, deliverables, tasks, and required resources.

Data Analyst

• Develop and maintain data processes and analytics related to the Centers for Medicare & Medicaid Services (CMS) -Hierarchical Condition Categories (HCC) risk adjustment model for the Medicare population
• Ensure that risk scores are accurately calculated and all updates from CMS are applied to the CMS HCC Risk Adjustment model
• Use a suite of tools to implement risk score calculation, develop actionable insights, develop risk score forecasts, and create dashboards/reporting, and optimize existing processes
• Calculate risk scores for each Honest member using CMS HCC risk adjustment model and maintain model to account for any changes made by CMS
• Develop SQL processes to automate risk score runs that integrate data from a multitude of datasets
• Evaluate project performance and identify value drivers for risk adjustment initiatives and programs
• Develop processes to calculate and automate key performance indicator (KPI)’s specific to risk adjustment as well as effectively communicate KPI’s to internal stakeholders
• Work closely with internal teams (Actuarial, Health Economics, and Finance) to provide data and insights necessary for financial forecasts and projections

Risk Adjustment Data Analyst

Accounting Manager

• Report to the Director, Accounting and collaborate to build a best-in-class accounting function
• Hands-on and highly visible role with direct responsibility for month-end close procedures, including revenue recognition, cash, accounts payable (A/P), journal entry approvals, financial reporting review, and monthly reconciliations
• Provide guidance and leadership to direct reports; allocate tasks and drive team performance
• Assist in managing general accounting functions, including accounts payable, accounts receivable, general ledger and the annual audit
• Establish and assess policies, practices and internal control procedures, and make recommendations for improvements
• Prepare, review, and analyze monthly, quarterly, and annual financial statements to ensure accuracy, completeness, and alignment with General Accepted Accounting Principles (GAAP)
