Assist Harmony Cares Primary Care Physician, Nurse Practitioners and Clinical Teams in the delivery of primary health care and patient care management in a residential setting
Support Patient Care through Clinical Partnership with Nurse Practitioners, Physicians, and Care Management Teams
Coordinates daily travel itinerary and transports clinicians to scheduled appointments
Prepares patients for examination and treatment; may include preparing patients for minor procedures
Secures patient consent forms and records patient care documentation accurately and in a timely manner
Coordinates patient care as directed by Clinicians, company standards and policies
Respects patient confidentiality and treats patients with courtesy and respect
Performs phlebotomy (venipuncture only), EKG or other procedures as requested by Clinician
Labels, stores and properly packages specimens for lab delivery
Assists in identifying and closure of quality gaps
Documents and updates patient care plans, SDoH and other assessments within the patient chart or electronic medical record
Maintain compliance with CLIA, OSHA, and HIPPAA guidelines
Prepare and maintain supplies and equipment for treatment (supply inventory, medical bag and car)
Ensure company vehicle is clean and proper periodic maintenance schedule is followed
Attends required meetings and in-services
Works with Primary Care Physicians, Nurse Practitioners, Clinical Teams, Patients and Patients' Families
Requirements
High School graduate or GED equivalent
Graduate of an accredited Medical Assistant, Phlebotomy, EMT, related medical education/training program or 1 year of experience serving as a Medical Assistant or Phlebotomist
Current CPR certification that meets Company guidelines
Knowledge of medical terminology
Basic computer literacy, including EMR, iPad, and GPS experience
Strong interpersonal communication and customer service skills
Must maintain a valid driver’s license and good driving record
Certified Medical Assistant (preferred)
Six months of experience in patient care, preferably geriatric care (preferred)
Strong Phlebotomy skills (preferred)
Desire and ability to obtain MA certification with 180 days of hire (preferred)
Full range body motion including handling/lifting patients
Ability to drive up to 150 miles daily in urban and/or rural setting
