Deliver annual risk assessment in a residential setting or telehealth within scope of practice delegated by Collaborative Physician
Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions and determine physical, mental, and social needs
Take history, examine, determine diagnoses and take patient vital signs as necessary
Provide written documentation of patient visit per NCQA standards
Place case management referrals and communicate with PCP as necessary
Communicate with patients, caregivers, agency nurses, other providers and vendors to assure proper diagnosis
Attend required meetings and in-services, participate in committees, and professional development activities
Requirements
Active/unrestricted nurse practitioner license to practice in coverage states
Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP) or National Commission on Certification of Physician Assistants (NCCPA)
Active CPR Certification
Current enrollment in Medicare/Medicaid
Must maintain a valid driver’s license and good driving record
Outstanding EHR skills
Geriatric training/experience (preferred)
Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public (preferred)
