About the role
- Partner with the Practice Operations team to strengthen, standardize, and operationalize key social work–led initiatives across the organization.
- Provide focused, short-term expertise in housing operations, emergency response workflows, and longitudinal social work care management and reporting.
- Work closely with clinical leaders, social work stakeholders, and cross-functional partners to translate social work practice into operationally sound, repeatable processes that support high-quality, compliant, and participant-centered care.
Requirements
- Master’s degree in Social Work (MSW) required.
- Active clinical or macro social work experience, preferably in healthcare, PACE, geriatrics, or complex care settings.
- Demonstrated experience translating social work practice into operational workflows, playbooks, or programs.
- Strong understanding of housing instability, community resources, and social determinants of health.
- Experience with care management models and performance or outcomes reporting.
- Excellent written communication skills, with the ability to create clear, actionable documentation.
- Comfortable working in a fast-moving, ambiguous environment with evolving priorities.
