About the role
- The Case Manager provides care coordination for individuals identified in High Utilizer Referral program.
- Work with High Utilizer Referral Social Worker, Mobile Crisis Staff and Suffolk County Police Department- Behavioral Health Section to outreach, assessment, and link individuals disproportionately engaging with law enforcement.
- Conference cases, recommendations and barriers to care with clinical staff.
- Track and coordinate outreach efforts, linkages and treatment compliance.
- Coordinate with other agency departments and community resources such as providers, housing agencies, DSS and Suffolk County Police.
- Engage families and other natural supports to identify individual’s needs, facilitate linkages and promote follow through.
- Utilize crisis de-escalation, motivational interviewing and harm reduction techniques to engage individuals in treatment and safety planning.
- Maintain documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedure.
- Maintain knowledge on available community resources, including but not limited to, medical, behavioral health, housing, DSS entitlements, transportation, vocational and other services that address concrete needs.
- Complete trainings and maintain proficiency with de-escalation and crisis intervention techniques.
- Abide by FSL’s Code of Ethics.
Requirements
- Bachelor’s degree required or Master’s degree in a related behavioral health field preferred (social work, mental health counseling, psychology, health administration or similar degree.)
- A minimum of 1-year related experience providing care coordination in a behavioral health and/or forensic setting and demonstrated experience in interdisciplinary care coordination required.
- Familiarity with NYS Entitlements, Housing, Health, and/or Mental Health/Substance Abuse Service Delivery System required; knowledge of Medicaid, Social Security and other entitlements preferred.
- Computer proficiency, including Microsoft office and Electronic Health records required.
- Excellent interpersonal and verbal and written communication skills required.
- Strong organizational and multi-tasking skills.
- Valid, clean New York State Driver’s License required.
