About the role
- Responsible for ensuring effective psychosocial intervention
- Manages behavioral and psychosocial needs
- Assists members to effectively utilize available resources
- Evaluates members' ability to independently manage self
- Provides guidance to members seeking alternative solutions
- Evaluates members' strengths related to health self-management
- Facilitates and coordinates behavioral health resources as individual member needs are identified
Requirements
- Requires MS (at a minimum) in Social Work
- minimum of 3 years of experience in case management in a health care environment
- Current unrestricted LMSW or LCSW (or equivalent) license in applicable state(s) required
- Bilingual (Spanish) or multi-language skills preferred
- Experience with social determinants/drivers of health is strongly preferred
- Experience working in a medical office (dialysis, oncology, etc.) is preferred
