About the role
- Support complex ESKD patients by navigating the healthcare system via medical record review and consultation
- Identify medical, social, emotional, and financial needs and implement appropriate interventions
- Work autonomously and with the healthcare team to coordinate and facilitate quality, consistent, cost-effective care
- Hybrid-Remote: telehealth work from home plus travel for in-person visits to dialysis clinics and nephrology practices within assigned geography
- Coordinate care transitions, manage complex/at-risk patients, establish treatment plans in partnership with care team
- Assess patients to identify unreported medical conditions or changes and address social determinants of health
Requirements
- Must live locally
- Passion for delivering care to patients with complex, chronic illnesses
- Flexible and willing to pivot quickly on priorities
- Current RN License required (BSN preferred)
- Minimum of 2 years RN experience required (healthcare performance coaching)
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
- Current driver’s license and willingness to travel within an assigned territory
- Intermediate computer skills and proficiency in MS Word, Excel & Outlook
- Home office with internet connectivity at a minimum of 1MB upload and 1MB download speed required
- Preferred: 2+ years Case Management or Chronic Care Management
- Preferred: 1+ years of utilization management experience
- Preferred: Spanish-speaking in some markets
