About the role
- Meet in-person with service coordinators, VR counselors, school administrators, and state disability agency staff on a weekly basis at their New York offices.
- Conduct virtual meetings via Zoom, email, and phone with service coordinators, VR counselors, school administrators, and staff from state disability agencies to provide updates on client progress, address service-related questions, and ensure ongoing alignment.
- Represent Daivergent with professionalism while communicating client updates, resolving issues, and aligning on service plans.
- Manage the full client lifecycle from referral through onboarding, contract authorizations, engagement monitoring, and service closure.
- Monitor paused, stalled, or disengaged cases and complete follow-up or case closure.
- Support the onboarding of new coaches by providing program orientation and practical guidance on delivering services in both individual and group settings.
- Provide state-specific compliance guidance to coaches regarding ACCES-VR service codes, documentation requirements, and data standards for NY.
- Provide ongoing support to assigned coaches by responding to service delivery questions, assisting with issue resolution and escalations, and reinforcing service delivery expectations.
- Submit detailed, accurate, and timely case notes for all client, school, and agency interactions using Daivergent’s internal systems.
- Work closely with Daivergent billing team to request authorizations and amendments for client service plans.
- Conduct regular audits and quality assurance checks of coach notes.
- Participate in regular case meetings with other Case Managers to review escalations, support complex cases, and ensure consistent service delivery practices across teams.
Requirements
- 2+ years of direct case management experience in a disability or special education context.
- Bonus for experience liaising directly with state vocational rehabilitation counselors.
- 3+ years of direct disability service experience , such as special education, job coaching, behavioral therapy, life-skills training, or related fields.
- High School/GED required .
- Bachelor’s degree in disability services, special education, counseling, psychology, social work, or a related field preferred .
- Technical & Work Environment Based within 15 miles of New York City: given the importance of our in-person school, family, and VR relationships, the ideal candidate is located near NYC.
- Comfortable working in a remote, technology-driven environment .
- Strong ability to document interactions clearly and maintain organized workflows within digital systems.
