Field-based position with an office at home; requires travel to meet in person with members around Tarrant/Dallas and surrounding counties 25% of the time
Conduct ongoing transition planning starting when the member turns 15 years old
Assure that teens and young adult members receive early and comprehensive transition planning to help prepare them for service and benefit changes that will occur following their 21st birthday
Use a team approach with the member, others in the member’s network, and the member’s service coordinator
Counsel and educate members and others in the member’s support network about considerations and resources for transitioning out of STAR Kids
Create a continuity of care plan specific to the member and member’s needs to ensure a successful transition to adult care
Consult with supervisor and others in overcoming barriers in meeting goals and objectives
Present cases at case conferences
Utilize interviewing skills to ensure maximum member engagement based on key questions and conversation
Requirements
Must be willing and able to travel 25% in Tarrant/Dallas and surrounding counties. Reliable transportation required. Mileage is reimbursed per our company expense reimbursement policy.
1 year of Service Coordinator experience required.
