Directs and manages the interdisciplinary team (IDT) to meet home health patient and family needs
Oversees Case Managers’ direction of patient care activities
Conducts interdisciplinary team meetings and reviews IDT documentation for accuracy and completeness
Oversees clinicians’ development and management of the plan of care, admissions, discharges, resumptions, transfers, re-certifications, evaluations, and visits
Supervises patient and family care, assessing appropriateness, continuity, service, and quality of care
Audits patient records in house and assesses quality of documentation
Ensures plans of care are updated to reflect new orders, interventions, and instructions
Communicates patient status information to the business office and physician to ensure continuity and quality of care
Oversees billing accuracy by ensuring correct frequency, supplies, and medications are ordered
Makes marketing visits and provides in-services to referral sources
Participates in on-call duty, performs patient admissions and visits as needed, and may carry an RN Case Manager caseload
May serve as alternate Executive Director, carrying out administrative and management functions in the Executive Director's absence
Directly supervises up to 45 team members, including interviewing, hiring, training, performance appraisal, disciplinary actions, and addressing complaints
Makes or delegates post-admissions calls and performs other duties as assigned
Ensures adherence to state and federal regulatory agencies and processes EMR documentation workflow as needed
Requirements
Associate or Bachelor's degree required
One (1) year of home health care experience required
One (1) year of supervisory experience preferred
Active and unencumbered licensure as RN, PT, OT, SLP, NP, or physician required
Must be a Registered Nurse, OT, PT, SLP, NP, or physician licensed in the state of employment
Current CPR certification required
A valid driver’s license and auto liability insurance
Ability to understand, read, write, and speak English
Must be able to work a flexible schedule to include nights and weekends
Must be able to travel
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
Ability to compute rate, ratio, and percentage
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
Ability to write reports, business correspondence, and procedure manuals
Strong written and verbal communications
Working knowledge of nursing principles, standards and applications
Knowledge of use of medical devices, ambulatory aides, assistive and resistive devices, and training
Knowledge of medical terminology
Ability to work with all types and levels of patients
Strong organizational and time management skills
Must be able to lift and move objects up to 50 pounds
Visual acuity requirements including close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
