About the role
- Connect hospitals with Home Health and Hospice teams for coordinated care.
- Lead implementation of care coordination activities for Home and Community Care services.
- Establish and maintain core relationships within the hospital system for collaboration.
- Ensure value-based operational direction and hospital leader participation in coordination activities.
- Participate in operational improvement meetings for all ministries within territory.
- Responsible for leadership and training of clinical liaison and CTA staff.
Requirements
- Clinical licensure appropriate to discipline (e.g., R.N./LPN/LVN, Registered Dietitian) or a Bachelor’s Degree in Social Work.
- 3 years Complex healthcare business project management experience required
- 3 years Documented prior success in managing healthcare teams, business development, or program management
- Previous business development experience in the state healthcare marketplace preferred
- Previous experience collaborating directly with healthcare executives and decision makers, physicians, hospital discharge planners and other health care providers preferred
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
- Ability to compute rate, ratio, and percentage.
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Demonstrated interpersonal skills and the ability to communicate effectively through strong written and verbal communications in English.
