About the role
- Responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation.
- Assess the quality of care provided to home health patients, families, and staff.
- Audit documentation, compile and review clinical performance data for quality and regulatory compliance.
- Participate in activities to support survey readiness and provide education to support clinical development.
- Own monthly Home Health Clinical Scorecard and ensure all reviews are completed and accurate.
- Analyze information gathered from audits and outcome data and share findings with home health leaders along with recommended solutions.
- Ensure QAPI is accurate and complete, compile quality data on quarterly basis and submit summary to Regional Executive of Clinical Operations for Governing Body reporting.
- Collaborate effectively with Home Health Regional Executive of Clinical Operations and senior leadership to assure clinical excellence.
Requirements
- Associate of Science in Nursing or completion of an accredited Physical or Occupational Therapy education program required.
- Advanced degree in Nursing, Physical Therapy, or Occupational Therapy preferred.
- Minimum of three (3) years of recent home care experience required.
- Minimum of one (1) year experience in home health QAPI or QA related field preferred.
- At least one (1) year of experience with point of service electronic documentation, HCHB preferred.
- Ability to understand, read, write, and speak English.
