• Provide clinical and administrative leadership to a regional team of Nurse Care Managers
• Promote integration of physical and behavioral health, with a focus on prevention and wellness
• Partner with clinical leadership to align DD Health Home services with broader organizational programs
• Support continuous quality improvement initiatives and ensure adherence to policies, procedures, and performance standards
• Oversee staffing, supervision, and professional development of nursing team members
• Facilitate regular team meetings, supervision, and coaching to support staff success
• Ensure accurate, timely documentation and compliance with all regulatory and program requirements
• Monitor enrollment and service capacity, supporting efficient transitions and continuity of care
• Collaborate with internal and external partners, including consultants and leadership teams, to enhance care delivery and staff training

Regional Nurse Director

Events

• Provide supervision, guidance, and evaluation for assigned prevention staff
• Support prevention coordinators in delivering training, education, and technical assistance to community coalitions and organizations
• Assist in selecting and implementing evidence-based prevention programs aligned with community needs
• Promote best practices in prevention, including the Strategic Prevention Framework, across staff and coalitions
• Observe and support staff in coalition engagement, training delivery, and community-based work
• Coordinate training opportunities, including training-of-trainer events for prevention programs
• Provide direct education and support to community coalitions and partners as needed
• Support coalition development activities, including agreements, funding applications, and reporting updates
• Review prevention messaging and materials to ensure alignment with best practices
• Collaborate with leadership to develop work plans, training strategies, and department-wide initiatives
• Participate in Prevention Resource Center meetings, trainings, and statewide collaboration efforts

Prevention Supervisor – Coalitions & Programs

• Provide supervision, guidance, and evaluation for assigned prevention staff
• Collect, analyze, and compile data to complete timely and accurate grant and contract reporting
• Support prevention staff in developing and implementing evaluation tools, including pre/post surveys and assessments
• Share relevant research, trends, and best practices to strengthen prevention programming
• Provide technical assistance to community coalitions and support community readiness and satisfaction assessments
• Utilize state and local data sources to help identify prevention priorities and target populations
• Review prevention messaging and materials to ensure alignment with best practices
• Coordinate annual initiatives, including Tobacco Retailer Education visits and Synar surveys
• Collaborate with leadership to develop work plans, training opportunities, and department-wide initiatives
• Identify and promote professional development opportunities for prevention staff

Prevention Supervisor – Reporting & Research

• Provide leadership, supervision, and professional development to Regional Nurse Directors and dietician services
• Strengthen onboarding, training, and competency processes for nursing care management and wellness staff
• Champion population health, quality improvement, and integrated care initiatives across Health Home services
• Review dashboards, reports, and workflows to identify opportunities for operational and clinical improvement
• Support staffing coordination, caseload coverage, and continuity of services during vacancies or leave periods
• Collaborate with Compass treatment teams and Health Home partners to ensure coordinated, patient-centered care
• Partner with senior leadership on process reviews, program optimization, and referral pathways
• Guide health promotion, wellness education, and chronic disease management initiatives
• Ensure compliance with state Healthcare Home guidance, program standards, and reporting requirements

Senior Director – Primary Care

• Provide leadership, supervision, and mentorship to Regional Nurse Directors, supporting consistent nursing care management practices
• Strengthen onboarding, training, and competency processes for nursing leadership and care management teams
• Champion quality improvement, integration, and population health initiatives across Healthcare Home operations
• Review dashboards, reports, and workflows to identify opportunities for improvement and operational alignment
• Partner with behavioral health programs (CPRC, SUD, outpatient, and others) to ensure seamless care coordination
• Support staffing coordination, caseload coverage, and continuity of services during vacancies or leave periods
• Facilitate leadership meetings, clinic visits, and in-person engagement to support alignment and communication
• Maintain compliance with state Healthcare Home guidance, program standards, and reporting requirements
• Contribute to staff engagement initiatives, quality assurance activities, and strategic projects that support program growth

Senior Director – Behavioral Health

• Provide leadership, supervision, and mentorship to Regional Nurse Directors, supporting professional growth and consistency across regions
• Strengthen onboarding, training, and competency processes for nursing leadership and care management staff
• Champion quality improvement, population health initiatives, and integrated care practices across DD Healthcare Home operations
• Review dashboards, reports, and workflows to identify opportunities for improvement and operational alignment
• Partner with Compass treatment teams, Easterseals Midwest, and Health Home leaders to support seamless care coordination
• Assist with staffing coordination, caseload coverage, and continuity of services during transitions
• Facilitate regular leadership meetings, team communication, and in-person engagement opportunities
• Maintain alignment with state Health Home guidance, program standards, and fidelity requirements
• Contribute to staff engagement initiatives, quality assurance activities, and special projects that support program growth
