About the role
- Develops, assesses, and facilitates complex care management activities for primarily physical needs members
- Evaluates the needs of the member, barriers to accessing care, focuses on member priorities
- Coordinates between the member and care provider team to ensure appropriate care
- Provides ongoing follow up and monitoring of member status
- Collaborates with providers to identify specialists and community resources
- Facilitates education to members and their families on care options
Requirements
- Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
- 2 – 4 years of related experience
- minimum of two (2) years of pediatric experience required
