💰 $27 - $48 per hour
About the role
- Assists in developing, assessing, and coordinating holistic care management activities
- Develops personalized service care plans/service plans for long-term care members
- Evaluates the needs of the member and recommends the best outcome
- Coordinates between members, caregivers, and care providers
- Provides resource support to members and their families/caregivers
- Monitors care plans and provides recommendations based on member needs
- Collects, documents, and maintains member information for compliance
- Performs home and/or other site visits as needed
- Facilitates education to members and caregivers on procedures and benefits
- Provides feedback on opportunities to improve care and service delivery
Requirements
- Bachelor's degree
- 2 – 4 years of related experience
- Current Hawaii Registered Nurse (RN) License
- 30 semester hours in a human services field or equivalent quarter hours (specific to Iowa)
- 2 years of prior LTSS and/or HCBS coordination for North Carolina Standard Plan
- RN or LCSW required for North Carolina Tailored Plan
- 2 years of youth/young adult experience required for Florida LTSS ECC
- driver's license and child and adult maltreatment check (specific to Arkansas)
