About the role
- Conducts comprehensive assessments of the mental health care and basic needs of individuals seeking treatment or support at CMHA-WECB Early Intervention Program for Psychosis (EI Program)
- Screening referred individuals to determine eligibility for the EI Program
- Redirecting referrals when more appropriate services within the community are required after reviewing referral and /or assessments
- Completes regular assessments of client’s mental health and wellness and communicates assessment results with client, natural supports and other team members when needed
- Using high level of nursing knowledge, critical thinking skills, sensitivity and good judgment when assessing clients and circumstances for the purpose of treatment and support
- Provides Clinical Intervention to support client needs
- Developing goal focused support/care plans based on assessment with client and other supports as appropriate
- Assisting client in determining priorities for goals and interventions
- Outlining interventions and activities to be carried out
- Coordinating involvement of other agency/program staff and outside resources
- Ensuring that interventions address the outcomes or goals as derived from the clients’ plan of care
- Ensuring each client has a crisis plan on file
- Provides nursing clinical consultation to other team members and community support workers particularly regarding medications, side effects and medical condition
- Incorporates health promotion and determinants of health into care plan goals and interventions
- Ensures a minimum workload of direct client intervention as negotiated with the immediate supervisor
- Ensures objectives of program are met
- Providing psychosocial rehabilitation, counseling and therapeutic support to client in achieving goals
- Uses critical thinking, good judgment and can be relied upon when problems or crises arise
- Follows college and agency guidelines when taking, recording and administering orders
- Participates in the evaluation of program goals and objectives making recommendations on changes to current program activities for improved service delivery
- Ensures all file management and reporting functions are maintained in a comprehensive and timely manner
Requirements
- Requires knowledge and skills acquired through completion of University degree. (B.Sc.N) in Nursing
- Current registration with the College of Nurses of Ontario is required
- Two years recent relevant clinical experience working with vulnerable individuals particularly those with mental illness or related diagnosis
- Comprehensive knowledge of mental illness and treatments; comprehensive knowledge of functions and principles of case management; thorough knowledge of psychotropic and other mental health related medications; up to date knowledge of relevant mental health and nursing policy, legislation and regulation; effective written and oral communication skills; knowledge of community resources.
- Bilingualism in both official languages at the advanced level is considered an asset
- A valid Ontario Driver’s License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.
