• Perform case management according to the Gap Housing Program, which offers preventative and homeless services.
• Develop written program guidelines that specify eligibility criteria, enrollment procedures for the housing program, length of stay, referral process, and client responsibilities.
• Provide case management in collaboration with the City’s Community Outreach Specialist, including assessment, intake, and referral support.
• Coordinate transportation to and from temporary housing, establish contact with housing providers, and deliver daily case management services to clients.
