About the role
- Conduct comprehensive assessments to identify strengths, needs, and service gaps.
- Develop, implement, and monitor individualized, family-driven Plans of Care.
- Coordinate medical, behavioral health, educational, and social services.
- Facilitate care team meetings and interdisciplinary collaboration.
- Support transitions between levels of care or service systems.
- Provide outreach and ongoing engagement with youth and caregivers.
- Educate families on available services, benefits, and treatment options.
- Advocate for access to medical, behavioral health, social, educational, and supportive services.
- Maintain accurate, timely documentation of services, assessments, and care plans.
- Conduct home visits, school visits, and community-based meetings as required.
- Collaborate with external providers, social service agencies, and care team members.
Requirements
- Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, or related field required.
- Minimum 2 years of experience working with children, adolescents, or families in behavioral health, social services, care management, or related field.
- Strong communication, organization, and problem-solving skills.
- Ability to work independently and collaboratively with multidisciplinary teams.
- Experience in care coordination or case management programs.
- Knowledge of child welfare, developmental disabilities, or behavioral health systems.
- Bilingual in Spanish preferred.
