The Registered Nurse Case Manager team at AptiHealth is pivotal to the success of our Case Management and Medication Management programs and provides a backbone of support to our patients and our providers.
Assist with assessing patient case management needs and implementing case management care plans.
Assist with implementing providers’ orders, administer and coordinate medications, procedures and special tests, and document treatment as required by company policy and local/state/federal rules and regulations as directed by the Clinical Director.
Order, interpret, and evaluate diagnostic tests to identify and assess patients' conditions.
Assess and evaluate patients' needs for, and responses to, care rendered.
Take live calls from patients calling into the front office with urgent medical/medication-related questions and triage these calls.
Work closely with a multidisciplinary team within the AptiHealth Medical Group to provide high quality care to each AptiHealth patient.
Requirements
Successful completion of an accredited nursing program, Bachelor’s degree preferred
Currently licensed as a Registered Nurse in New York
Current Certified Case Manager (CCM) certification preferred
Experience in behavioral healthcare a plus
Strong computer skills and experience with Electronic Medical Record systems
Experience with Microsoft programs including Outlook and SharePoint Forms preferred
