• Outreach to patients to provide encouragement, check-in on goal progress, problem solve, and provide support
• Review patient status and escalate urgent matters as needed to care team providers

• Deliver high-quality, virtual individual counseling to a diverse patient population
• Provide a minimum of 30 clinical hours of individual therapy per week
• Utilize the aptihealth telehealth platform to deliver engaging, secure, and effective virtual care
• Develop personalized care plans using aptihealth’s tools and technology
• Monitor patient progress, adjust treatment plans as needed, and document clinical findings
• Collaborate with multidisciplinary care teams to ensure coordinated and holistic care

Manager

• The Registered Nurse Case Manager team at AptiHealth is pivotal to the success of our Case Management and Medication Management programs and provides a backbone of support to our patients and our providers.
• Assist with assessing patient case management needs and implementing case management care plans.
• Assist with implementing providers’ orders, administer and coordinate medications, procedures and special tests, and document treatment as required by company policy and local/state/federal rules and regulations as directed by the Clinical Director.
• Order, interpret, and evaluate diagnostic tests to identify and assess patients' conditions.
• Assess and evaluate patients' needs for, and responses to, care rendered.
• Take live calls from patients calling into the front office with urgent medical/medication-related questions and triage these calls.
• Work closely with a multidisciplinary team within the AptiHealth Medical Group to provide high quality care to each AptiHealth patient.
