Develop and manage personalized discharge care plans
Coordinate services across providers and community resources
Educate members and families on diagnoses, medications, and self-care
Monitor progress and ensure continuity of care
Advocate for member needs and support their recovery
Requirements
Active, unrestricted California RN license
Bilingual in Spanish or Vietnamese
2+ years of case management or clinical experience
Willing to obtain licensure in other designated states (Non-compact: NV; Compact: AZ, NC, TX) within the first 6 months of employment (licensure fees reimbursed by the company)
