About the role
- Coordinate patient care and ensure patients receive appropriate services in a timely manner
- Advocate for patients and manage acute care situations
- Conduct patient assessments and develop/implement case management plans
- Manage discharge planning, prior authorizations, DME orders, and appeals/denials
- Perform utilization review and care coordination across interdisciplinary teams
- Ensure compliance with CMS, HIPAA, DRG, The Joint Commission, and Worker’s Compensation requirements
- Work in acute hospital settings including ICU, MS, SDU/PCU/IMC/Obs
- Day shift (08:00 - 16:30), 40 hours per week, 13-week contract, in-person
Requirements
- 2 years of experience
- Travel experience required
- BLS certification
- Previous charge experience preferred
- Experience in Acute Hospital Case Management/Utilization Review
- Experience with Appeals and Denials and Care coordination
- Experience with Disability case management
- Experience with Discharge Planning, Needs Assessment, Order DME, Plan of Care, Prior Authorizations
- Knowledge of CMS, Department of Health, DRG, HIPAA, The Joint Commission/Core Measure/National Safety Goals, Worker’s Compensation
- Acute hospital experience in specialties: ICU, MS, SDU/PCU/IMC/Obs
- Ability to work Day shift 5x8-Hour (08:00 - 16:30)
