About the role

  • Coordinate patient care, ensuring effective communication among healthcare providers.
  • Facilitate optimal health outcomes for patients through case management.
  • Manage discharge planning and utilization management processes.
  • Document and maintain patient records using Epic charting system.
  • Provide care across infectious disease, outpatient, and emergency medicine settings.
  • Travel as required to support patient care and case management.

Requirements

  • Travel Experience Required: Yes
  • Current Registered Nurse (RN) license in the state of practice.
  • Proven experience in patient care, preferably in medical-surgical or primary care settings.
  • Knowledge of infectious disease care protocols and practices.
  • Familiarity with utilization management processes and discharge planning strategies.
  • Strong understanding of medical terminology relevant to nursing practice.
  • Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare teams.
  • Ability to work collaboratively in a fast-paced environment while managing multiple priorities.
