About the role

  • Seeking a Full-Time Home Health Physical Therapist to service our Home Care Patients in the Coopersville, Walker, Allendale and Muskegon areas.
  • Accurately assesses clients at admission and other required time points and utilizes appropriate standardized documentation.
  • Assesses the client’s condition and needs and determines the agency’s ability to meet those needs in the home care setting.
  • Informs the client/caregiver about the agency, care/services available, client obligation for payment, and rights and responsibilities before care is initiated if therapy only case.
  • Makes appropriate referrals for evaluation/care to other disciplines and services, and coordinates care with others to ensure effective and efficient care is provided.
  • Reports changes in client condition as appropriate and in a timely manner, to the client’s physician and obtains orders for changes in the plan of treatment.
  • Initiates and implements appropriate preventive, therapeutic and rehabilitative procedures, and techniques to clients.
  • Performs manual therapeutic exercises, massage, and/or manual traction as established by the plan of care.

Requirements

  • Possession of a degree, which includes an internship from a baccalaureate/university program in physical therapy approved by the American Physical Therapy Association.
  • Licensed to practice as a physical therapist in the state of practice.
  • Must have a minimum of one (1) year experience as a physical therapist or have a preceptor for a minimum of 6-8 weeks.
  • Current working knowledge of conditions of participation and home healthcare experience preferred.
  • Must be able to function in an environment with minimal direct supervision, accepting personal responsibility for maintaining professional relationships with agency’s clients and families.
  • Must have current Driver’s license and reliable transportation to and from work site.
