About the role
- Provide direct supervision and administrative oversight for a localized, Integrated Care Team supporting Innovations members and members without Innovation waiver funding living with mental health, substance use, and intellectual/developmental disabilities.
- Ensure staff utilize workflows as required to complete Care Management Comprehensive Assessment and care plans for members as care needs change.
- Establish a team based, person-centered approach to coordination of care to be implemented by all team members that effectively manages a member’s physical health, behavioral health, and social determinant of health needs using established workflows and processes effectively and efficiently.
- Monitor teams to help ensure integration of care through the establishment of an integrated care team (e.g. member, caretaker/legal guardian, family member, PCP, behavioral health provider, specialists, nutritionists, pharmacy, etc.) to meet the member1s needs.
- Use knowledge and skills to create and identify methodologies and tools to be used in the execution of care management and coordination.
- Coach team members to use member engagement practices that support the engagement of members throughout the continuum of care.
Requirements
- Associate1 degree in Health, Psychology, Sociology, Social Work, Nursing, or another relevant human services area and a minimum of three (3) years of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI.
- OR Bachelor1 degree in Health, Psychology, Sociology, Social Work, Nursing, or another relevant human services area and a minimum of one (1) year of experience providing care management, case management, or care coordination to complex individuals with I/DD or TBI.
- OR Equivalent combination of education/experience.
- Preferred Experience: Supervision of two (2) or more employees preferred.
- Prefer experience focused on working with Innovations in the mental health, substance use, and/or intellectual/developmental disability and TBI populations.
- License/Certification: Must have a valid driver1 license
