About the role

  • Theoria Medical is a comprehensive medical group and technology company dedicated to serving patients across the care continuum with emphasis on post-acute care and primary care.\n
  • Theoria’s Clinical Operations Department seeks an experienced Director to oversee CCM program, including supervision of clinical staff, program development and evaluation, and coordination with other healthcare providers.\n
  • Shift Structure: Monday-Friday 9am-5pm EST Remote.\n
  • Employment Type: Full-time, Exempt.\n
  • Essential Functions & Responsibilities: Oversee and manage the day-to-day operations of the CCM program; Supervise and mentor the clinical staff, including pharmacists, nurses, and other healthcare providers; Develop and implement strategies to improve the CCM program; Coordinate with interdisciplinary teams on patient care; Participate in tech development projects to streamline CCM processes; Ensure adherence to regulatory guidelines and maintain up-to-date knowledge of developments in the field; Analyze patient data to guide improvements in the CCM program.\n
  • Requirements and Qualifications: Bachelor’s degree in Healthcare Administration, Public Health, Nursing, or a related field required; Master’s degree preferred; Experience overseeing a chronic care or similar program; Strong leadership and management skills; Excellent interpersonal and communication skills; Strong computer and technical skills; experience with telehealth platforms preferred; Understanding of the role of a CCM program in healthcare delivery.
