About the role

  • Coordinates patient documentation to maintain eligibility for a variety of core medical and social support services;
  • Conducts patient interviews required to complete intake applications and all required forms for core medical and social support services;
  • Works closely with medical case managers to reduce patient barriers to obtaining needed medical care and /or social services;
  • Assist with the identification of community resources that build support for medically and socially complex patients;
  • Maintain current information on all regularly accessed community resources;
  • Perform a range of administrative functions required to ensure efficient program operation.

Requirements

  • High School diploma or GED and 2 years of related experience or a certification as a Certified Community Health Worker (CHW).
  • Verification of education and licensure will be required if selected for hire.
  • Required License: Possession of a valid unrestricted NM driver's license.
