About the role
- Provide clinical leadership, supervision, and operational oversight for a team of Behavioral Health Care Managers
- Balance direct patient care, quality assurance, and cross-functional collaboration
- Support onboarding and training of new Behavioral Health Care Managers
- Monitor team performance against key CoCM metrics
- Ensure team adherence to CoCM clinical workflows, documentation standards, and best practices
- Identify trends, risks and opportunities for improvement across the patient population
- Maintain a patient caseload, as needed, to support team coverage and model fidelity
- Support mental and physical health of patients on an assigned caseload
- Participate in regularly scheduled caseload consultation with the Psychiatric Consultant
- Screen and assess patients for common mental health and substance abuse disorders
- Provide education about common mental health and substance abuse disorders
- Track treatment response and monitor patients for changes in clinical symptoms
- Adjust treatment plans for patients who are not improving as expected
- Facilitate referrals to evidence-based psychosocial treatments
Requirements
- Masters degree in social work or other behavioral health related degree
- Specialized training to provide brief psychosocial interventions, motivational interviewing, and problem-solving treatment required.
- Minimum of 4-6 years of experience in behavioral health, integrated care, or care management
- At least 1-2 years of clinical supervision or team leadership experience
- Licensures and educational requirements include but are not limited to:
- Licensed Mental Health Counselor/Professional Counselor
- Licensed Marriage and Family Therapist
- Licensed Social Worker
- Licensed Psychologist
- Demonstrated ability to collaborate and communicate effectively in a team setting.
- Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
- Experience running a clinical team.
- Experience scaling CoCM programs across multiple practices preferred.
- Experience in virtual delivery care models.
