About the role
- Initiate, modify and discontinue medications within a Collaborative Practice Agreement
- Focus on management of complex disease states to help patients reach optimal therapeutic goals
- Simplify patient medication regimens, minimize harmful medication interactions, and educate patients
- Responsible for policy development, Continuous process improvement, continuing education
- Conduct patient care assessments, orders and interprets laboratory tests
- Prescribes, modifies and discontinues medication therapies
- Monitors clinical outcomes and communicates with medical providers
- Performs electronic documentation and refers patients to other health care providers
Requirements
- Graduate of an accredited school of pharmacy in the U.S
- 4 years recent relevant experience
- Oncology pharmacy experience (dedicated role) Preferred
- PHARMR-Registered Pharmacist Upon Hire
- ASHP-Post Graduate Year 1 ASHP General Pharmacy Residency Upon Hire
- ASHP-Post Graduate Year 2 ASHP General Pharmacy Residency, Oncology Preferred
- Advanced Practice Pharmacist (APh) license Preferred
- Hospital Medical staff Credentialing required, to be completed within 6 months post-hire date
